In my opinion, if you saw Chapter 25, it would be kind of hard to miss, since Grievous was fine since Chapter 20, and as soon as Mace hits him he falls and starts coughing...
Whether or not you know the plot reason, I think it should be kept in. After all, if Grievous is a foreshadow of Vader, than he coughing should be kept in since it's a foreshadow of Vader's breathing.
