Footage wasn't missing. However there was 2-3 seconds of blackness during the flip between LD sides.
Rowman "recently" made it seamless. Could that be the fix you're thinking of ?

Even more recently Wookie Groomer made it seamless too, and this set ROCKS !

Several of the Quality Broth set's LaserDisc side changes are sloppily done.
For example at the end of Side 3, when Luke reads the trash-compacter's serial number
to Threepio, the last digit "seven" gets cut.
Earlier at the end of Side 1 after Owen's "There'll be hell to pay"
there's a bit of footage missing before Luke's "There's a droid on the scanner."
In ESB, as Luke enters the carbon-freeze chamber and Artoo gets locked out in the hallway,
there's a bit too much "dead air" between the LD sides, making the transition noticeable.

The LD side changes seem to match the '93 Definitive Collection (albeit, a later pressing with Leia welding).
The Quality Broth .NFO files don't say which LD version was used, only "THX".
Grisan, why do you say '95 Faces was the source ?

(chapter timings calculated from frame numbers in Chainsaw's hyperlink )

13 The Millennium Falcon to the rescue (370 frames = 15 sec)
14 Luke's shot (284 frames = 12 sec)

are combined on Dr_Gonzo's DVD chapter 74 (0:27 sec)
while

01 Opening Logos - "A long time ago..."
02 Main titles - A New Hope

are combined to form Chapter 1 of Dr_Gonzo's DVD.

So because of these two combinations, Dr_Gonzo's DVD has only 79 chapters
while the inserts (thanks everybody for posting them !) list 81.

Make sure you have IFOedit version 0.95 or later (better command editing interface). Make a backup of the VIDEO_TS.IFO file (in case you mess up), and open the file in IFOedit.
