This is the worst DVD cover art I've personally come across.
I remember seeing an a even worse cover for the Teenage Mutant Ninja Turtles movie a few years back, but I can't seem to find a copy of it on the 'net.
This thread is for sharing any horrible cover art you've come across. I really can't fathom why studios decide to use absolutely terrible slapped together junk instead of the original poster art. I know sometimes it's because they're double dipping and want to differentiate between releases but I've seen more than my fair share of crap covers on the first release.
I have no words:

My god... 3 and 1 don't look quite so awful, but 2 genuinely looks like a 12-year-old's photoshop job!

That isn't just bad, that's plain lazy! They look like really bad temp pictureholders for Amazon or something!
The new one for Strangers on a Train was pretty lackluster but nothing compared to that above:

But seriously, most covers from older films which originally have amazing poster art have some crappy Photoshop cut/paste stuff, ugh. Even popular movies like Bond films, most recently they just had some white background and then two persons pasted onto it.

the quality of the cover art never matters that much for me. I care more about what is on the disc(s).

Yes I have, though part of me wishes I hadn't.
Of all the optical disc packaging I've encountered in my travels, this was *the most*... execrable.
Packaging that actually provokes damage not only of itself but also of the discs it holds.

The guy in the video makes it look much more easy than it actually was in my case.
The clam shell was very tightly locked and pressure had to be applied in specific points to pop it open (which in my case caused the hinges to fly apart and scattered the discs and the little plastic menu cards all over the floor).
The titles on the double sided discs are quite difficult to read too (lucky for me that HDDVD never really took off as I've never had to use that side which gives me a optional 'finger touch' side).
And I complain because the liner notes are on the inside of the cover of the Blu-ray and I have to take the last disc out to see what's on it.
I also complain because the second and third season don't remember where I left off so I can't start and stop episodes when I please. But that's not related to the topic.

Wow... Those are beyond terrible. It's like a parody of all those "Two Big Heads" covers. Given the popularity of the films, the fact that it's a big studio product and the resources at hand, I'd say these are clearly the worst covers I've ever seen.
