There really isn't another answer. Only the apologists fail to see it. In fact, they enabled him and allowed him to continue by not calling him on it.

For example, Vader runs around calling himself Darth in the OT. Apologists will claim that most people in the galaxy don't know what a Sith is. That would be like most Americans not knowing who the British are.

My best guess, in-universe, is that although such things would be possible and easily doable, mass media isn't a strong force for cultural reasons. It just gets lost amidst a galaxy of voices.

I wouldn't mind clues from future books that help mend these oddities, like Darth Plagueis, weaved into a compelling story.

I know I’ve made some very poor decisions recently.
