Perhaps it is the output codec you are using, some of those screw you up. It could also be that the NTSC IRE black levels are causing you problems, you have to be careful there.

On the PAL at PAL speed in NTSC playback question, the only thing I can think of is that the player could do a 3:2 pulldown every 5/6 frames instead of every 4/5 frames...

If you're using TMPGEnc for encoding, you can do a manual 3:2 Pulldown reversal frame-by-frame. This is what I had ended up doing long ago.

That's one thing I haven't found since switching to OS X - a good manual 3:2 Pulldown removal option.
