It was mostly substance, especially the conversation before and after the duel. The ‘flash’ is actually symbolic.
The lava symbolizes Anakin’s spiritual descent into hell.

Obi-Wan: You were the chosen one! It was said that you would destroy the Sith, not join them. You were to bring balance to the force, not leave it in darkness.
Anakin Skywalker: [shouting] I hate you.
Obi-Wan: You were my brother, Anakin. I loved you.

Dueling with a life-long friend is a more emotionally grueling experience than dueling with a long-lost father with whom you have never had a relationship.

Obi-Wan’s duel with Anakin in Revenge of the Sith is also more emotionally powerful than Obi-Wan’s duel with Vader in A New Hope because in that film his past relationship with Vader is only spoken of, never shown.
