Not a printing problem. I found out it was in the initial design phase. The guy who designed it has used the photo before and tried to make it appear different by simply reversing it. This is usually a good way to do it, but not here.

It's a case of both being lazy and falling into a routine. When you proofread enough stuff, your eyes and brain gloss over a bit. Usually, you just look at text, so not noticing a photo can be explained that way... you are so worried about content, you don't look at design.
