The ripped video stream seems to be correct about 98% of the time, but several times over the course of the film, some frames are duplicated, and then later on, some frames skip, bringing it back into sync.

 

Out of curiosity, have you tried working on it in smaller, uncompressed files, then stringing them all together with mkvmerge or something similar? I don't know what your hardware is like, so I'm just speculating, but I can imagine Premiere choking pretty hard on such a lengthy HD video file -- particularly one that's already compressed. Does the demuxed h264 stream play correctly in a media player outside of Premiere?
