I noticed that on some early HD shows at the time. I am not sure what is going on but I think it is something that happened when people who were use to shooting on film made the switch to digital HD without much training. You are right everything including effects shots looks a little off. Sorry I have no idea what is going on but I think I know why it happened. You had a bunch of people who were used to working one way being forced to learn how to work another way on the fly and that can lead to a few hick ups along the way.
