The Sisterhood of Karn owed The Doctor for stopping the Time War, so when The Doctor was finally ready to retire from his adventures in space and time he returned to Karn and the sisterhood wiped up some regeneration brew that allowed the Doctor to regenerate once again into his Tom Baker form.
