I just looked into this. There are just a couple areas that can't be fast traveled to that can be hard to find. DLC doesn't count at all toward the percentage toward completion.
I've got all the pickups, camps etc in the game 100% but it's getting the achievement trophys that give a game replayability and two of them are for buying uprades online. Needless to say I didn't buy them.
Another example would be the otherwise excellent 'Arkham City' that has parts of the actual game missing (Catwoman, Robin etc) if you've don't purchase them seperately online or buy the right exclusive version at the store. Not to mention day one maps, skins, weapons, challenges.
I love it when (Like with 'Fallout') expansions get released 6 months later to extended the game. No problem paying for that extra content. But increasingly games are finished, have 10% cut out of them then are released. Then you are expected to pay again to buy back that 10%.
The way to do it is like with 'Prometheus', where there was a bog standard edition with more less just the film and a deluxe one with all the content. I happily paid the extra for the deluxe release and spent hours enjoying it all.
