If you look closely at the eyes, you'll notice that the "horrified" look is probably accidental, as I think at least one of his eyes was tampered with for the photo. Notice his right eye is more normal looking, his eyelid is in a "natural" position. Now look at his left eye: methinks it was artificially held open.

Judging by the early age and the age of the photo itself, I think a good bet would be some kind of nasty fever.
