If the DLC isn't installed, the game ends with you dying in the chamber. Unless you made the Brotherhood of Steel girl go in there to take care of it for you.
Actually I forgot to mention this. As I have mentioned, I've been rolling the Wasteland with my gang- my obnoxiously talkative Mute Ghoul Servant Charon, and my faithful dog(s) Dog Meat's Puppy(ies). When I went to vault 88, we were friendly to Fawkes because that's how my gang rolls. (We be bein' kind to evry foo' who cross our path!)
When I got near the end of Raven Rock... I'd realized I had passed through a one way door without grabbing the bobble head. So I had to reload and play it again. While looking for said bobble head, I accidentally passed through the one way door again- reload, try again. Eventually I got it, got out of Raven Rock and found Fawkes waiting for me, accepted him as a new companion (I'd lost Dog Meat's Puppy and Charon when they captured me). I figured I'd go look for Charon and DM's Puppy after I dropped off the last 2 bobble heads at my Megaton appartment. But then I had go to bed. To save time, I googled "Where are my companions after Raven Rock" and found out that the game wouldn't let me keep both Fawkens and Charon. I was really torn... because I have (in my mind) an awesome relationship with my mute ghoul servant Charon and we have a lot of history, but Fawkes has a gatling laser. Luckily I saw that you can cheat and get both if you fast travel away from Fawkes before he joins your gang (I call it the "Nice to Everyone Friendship Gang of Death") go get Charon (or whomever) and then come back to Fawkes. I didn't have a save right there, so I had to play Raven Rock. Again! Get the bobble head, defeat President Eden, blow up a lot of soldiers, and the rest.
But I have a 4 person (alright- 1 person, 1 mutant supermutant, 1 mute ghoul, and 1 puppy) gang now and I'm pretty stoked about that.
But the point of all of this- I didn't die nor did I have the chick die at the end. I just sent my mutant supermutant in to handle it. He was glad to do it and the radiation didn't hurt him any.

If you have the Broken Steel DLC installed, then as you saw, they retcon the ending and have you wake up from a coma. If you beat the game without any DLC installed the first time like I did, then once you install Broken Steel you have to load a save right before the end of the game (which I had), or play back up to that point to access the DLC which automatically starts right after the ending.
They rolled the "end video" then came up with the 2 week's later card. Apparently Sarah and I were still in bad shape, but neither of us died. Can I assume the "have Fawkes do it" option was open before the DLC? Or did they add that so that neither of us had to die?
What did you do with the modified FEV? I didn't think it sounded like such a bad idea, but I sort of accidentally didn't mean to give it to Elder Lyons. And I would have totally forgotten to do anything with it at the end, despite having looked at the injection point a couple of times earlier in the game. Kill all of the feral ghouls and supermutants whilst purifying the water? Sounds good to me! Feeling like maybe I should play Raven Rock (again!) I turned to google again to see what I was missing. If you didn't use the FEV or haven't read about it, I recommend you do the same. I'm glad I didn't do it, but I was impressed with the ramifications.
