In the old PC flight sim, X-Wing, you could eject from your ship and be treated to a cut scene of a Rebel ships coming and rescuing your pilot who is free floating in space (appearantly with some sort of sealed oxygen mask). I think in one of the Thrawn books I seem to remember Luke's X-Wing breaking down somewhere out in space, and he had to exit the ship to work in it, which required him putting on some kind of helmet that attached to his suit. Perhaps pilots are, ridiculously, just expected to quickly swap their fighter helmets out with this other mask when they see impending peril imminent* and press the ejector button.
* So much fun using those three words together!
