You've mentioned something that has bugged me about ESB for a long time.
That's Vader's two cries of pain and surprise during the duel. They were embarassing to hear. Vader's a guy who has clearly suffered consistent pain of every degree imaginable for years.
A little nip on the shoulder would have no effect on him whatsoever. At least not to the point that he made any sound out of it.
