Sure sounds like Dr_Gonzo's ESB disc with "Scoundrel" Easter egg.
Sounds like it's not Dr_Gonzo's cover though if it lists extras which aren't on the disc.

Have a look at russs15's site which has Dr_Gonzo menu screenshots and a whole lot more.
