If it's still like that on the DVD, is it an easy fix to flip it?

Lucasfilm fixed that for the DVDs.

Shouldn't Clive Revill be left in the credits somehow?

He's not in the movie any more... besides, I pasted Ian McDiarmid's name over his. I'd have to jiggle the whole credits around to add another name.
