I love a combination of Photoshop, Avisynth and DVDMaestro. Photoshop to create the non moving parts. Avisynth to add the moving parts to the stuff made in Photoshop. And DVDMaestro to add the menu to the dvd plus making the controls work.

Making a menu with these programs, is not the most easy way, but it gives you full flexibility.
