Wearing “thongs” (plural) are footwear that stays attached to your foot via a “thong” between your toes. We call them “thongs” in Australia and I believe Americans call them “flip-flops”, just like many other things have different names in different countries for the same thing.
Wearing a “thong” (singular) is a piece of underwear that incorporates a “thong” that goes between your butt cheeks and this element is used as the article name to clearly differentiate between it and normal “underwear” since the thong is the differentiating factor. Technically you could wear more than one in which case you could say you were wearing “thongs” but I would imagine this to be rare if not nonexistent and therefor is not related to the word “thongs”.
Sandals don’t use a thong to attach to your foot, instead just having a big loop or an ankle strap as well to properly secure it to your foot. Some sandals can also employ a thong but along with the other elements that make it a sandal. If all it has is the thong, then it is a thong and not a sandal.
