I encode to 23.976fps and then simply "pulldown film.m2v NTSC.m2v" which gives me a 29.97fps mpeg2 file with 3:2 pulldown flags in it. Having the pulldown.exe add the pulldown flags into the file means on a truly progressive setup (PC playback) you get smooth 23.976fps playback whilst on an NTSC tv you get the 3:2 pulldown 29.97fps playback.
