Its my understanding that at the time when 'Star Wars' was made, GL had no idea (or plans) to make another film. At that time, the events of the film should be taken at face value- no idea that Darth was Luke's father, no thoughts about Luke and Leia being siblings....

That said, when SW became the blockbuster that it did and GL saw that sequels were wanted by the fans and studio alike, he was able to flesh out the story and came up with most of the ideas that he has stuck with over the years.

By ESB he decided that Darth was Luke's father... but *still* had Leia lay a good snog on Luke early in the film which helped to set up the romantic tension later on with Han. Though we may never know for sure, I personally think that GL had NOT decided that Leia was Luke's sister at that time and *may* have had some other character in mind when Yoda and Ben mention 'another'. As to who, anyone's speculation is as good as mine.

By the time Blue Harvest...uh, I mean ROTJ ( ) was in production I feel that to wrap up the story in more of a clean fashion GL decided to just make that 'other' Leia. That way, no new character to introduce and a way to put someone Luke cares about in danger.

I realize fans have decided that the Force was able to shield Leia from Vader noticing her... but really: in SW she was directly under Vader's nose (including being interrogated... and one can presume that though neither of them knew her true origins Vader still probed her mind with the Force and probably doped her up in hopes of getting the location of the hidden Rebel base) and in ESB she, Han and Chewie were Forced (pun intended) to have dinner with Darth on Bespin.

So I feel the family additions were just as progressive as the whole movie-making process.
