I used to do some prank calls too. The best approach if the person get suspicious, is to pretend you know the person:

"Haha I suppose you know who's talking now, eh?"
"No I don't, who is it?"
"Guess who it is? C'mon... I was on the last family reunion..."
"Is it <name>?"
"Yes! It's me!"
"Dosen't sound like you."
"It's this cold cough cough, I have this funny voice because of my cold, and I decided to call you... So, how's everything."

And when I couldn't keep myself from laughing, I'd hang up in my favorite way:

"OH GOD NO! MY LEGS! MY LEGS! BURNING LIKE FIRE!!"
"What? What are you saying?"
"OH GOS THIS CAN'T BE HAPPENING! HELP! HELP! PAIN!!" *hangs up*
