Sometimes lots of it.
Carrie isn't a full time career actress, because of the sexism in Hollywood hardly any actresses her age are.
She has to take medication and that effects here weight in the real world.
Infront of the camera there will be teams of people making sure every actor looks like their character.
Did you notice how fluffed up Ewan was in ROTS?
They had about three people just to get his hair looking right.

I am ready for the trials!We all try to look our best when a photo is taken. (In theory.) Unfortunately for actors and other famous people, paparazzi are literally under every rock. Pictures are taken every day of the week, every moment out in public, healthy or sick, bad hair day, happy or sad. And plenty of sleazy tabloids and website to buy those pictures.
Think of the worst day you've had lately, and imagine someone has pictures of you from it, and is passing them around.
